Application Forms are obtained from the Nursing Service Department, Ministry of Health, New Secretariat, Umuahia on payment of non-refundable fund fee of Ten Thousand Naira (N10,000.00) only at UBA and First Bank.

Purchase of forms begins on 7th December 2020 to 15th January 2021 (6 weeks)

All completed forms should be returned to the Nursing Service on or before 15th Friday, January 2021
