Step 2: Pay the Application Fee

Applicants must pay a non-refundable application fee of ₦20,000 through Remita.

Payment should be made to:

  • UCH TSA Account

  • For School of Occupational Health Nursing

After making the payment:

  • Print out the payment slip

  • Take the slip to the UCH Accounts Department

  • Convert the payment slip into an official receipt

This receipt is very important and must be kept safe.


Step 3: Download and Complete the Application Form

Applicants should:

  • Download the Applicant User Guide from the portal

  • Carefully read the guide

  • Complete the application form online

All information provided must be correct. Wrong details may affect the application.

After completing the form, submit it online through the portal.


Step 4: Print Application Documents

After submission, applicants must:

  • Print two copies of the completed application form

  • Print the photocard

These documents will be used for authentication and verification at the School of Occupational Health Nursing.


Step 5: Pay Additional Council Fee

Applicants are also required to pay ₦200 to the Oyo State Nursing and Midwifery Council Committee Account.
